Webform Edits

Hello,

I would like help adding additional questions to our webform. The survey is to collect child participant data for the Infant and Child Developmental Group’s Database. I am unable to edit the webform and need help. We need the following questions (same format as Gender question):

Race:

• Black/African American
• Asian
• Other

If other, please explain below:
