reCaptcha error: invalid domain

Issue/Request: 

The webform on this URL is showing the following reCAPTCHA module error:

Users are reporting they can’t submit the webform due to the error.  I can’t figure out where within the webform the reCAPTCHA prompt is located in order to see if I can fix the domain. 

Please advise.  Thanks for your help.
