Problem with webform

Hi, we’re trying to set up a very basic form but run into problems. First, the page above is a webform added as a page. We’d like it to be a part of another page, at the bottom of more general information. But we just can’t seem to just add a webform on a page.
Secondly, when we tested the form by submitting it to my address,
Yale School of Management came as the sender. I have no affiliation with SOM, so we’re wondering where this is set.

Hi Bjorn,
For the first point, you can make the Webform available as a Block which you can then place on a specific page. You can do this by editing the Webform and go to Form Settings > scroll to the bottom of the page to Advanced Settings and select the “Available as block” option then hit save. You should then see the block in the Blocks admin page and you can place it in the Content region of the page that you want.
I also suggest that you add reCAPTCHA to this webform to avoid spam submissions.
For the second point I’m not able to reproduce the SOM sender, I’m seeing it come from Judaic Studies Program as it’s configured.
