Need to hide information at top of webform

Issue/Request:

At the top of the web form that we have created on the site is authoring information that doesn’t need to be there. Can you get rid of it for me? I tried, but to no avail. This is not an emergency and I will launch the site even if this is not fixed, but please let me know where to find how to fix it or tell me and I will do it. Thanks.

It’s a check box in the Content Type. I have unchecked it for you.
