Issue/Request:

We would like a feedback button, available on any page, that users can submit comments or questions to us. Many of our pages contain help information and many users want to be able to submit corrections or additions. Page comments don’t really work for this. A feedback button linking to a form input would be ideal. The form would send us email and contain information about the page they are looking at (e.g. the page name). Does anything like this exist already, possibly as a module?

There is a web form module: http://yalesites.yale.edu/module-tutorials/webform, that you should take a look at. I think you could get it to do what you want. Let me know if this works for you.

Yeah, the form is OK, but I’m actually looking for a button that will launch the form. The button should capture information about the page on which the button is and send it to the form (in a hidden field possibly) that can be sent in. Does anything like that exist?

At present, YaleSites does not have an application that has the functionality you are asking for. The type of application you are asking about would have to be custom built.
