I need to reconfigure that “specialities” menu – I don’t want it to be expanded, just one row across with no default choice, AND I need to reorder the items and add a couple other choices in that dropdown.

I’ve done some research into your request. The way the multi-select box widget looks now is how this module is designed, if you want the user to be able to select more than one choice. At present, there is no simple option to change this.
