Bug with Short Answer question type and quiz module

Hello,

I’m trying to set up a quiz on the website http://dev.variationsstylistiques.yale.edu/chapitre-1-quiz, and have been having some trouble with the short answer question type. Whenever I submit an answer to a short answer question and then try to move on to the next question (or save my score), I get an error message about an undefined index (see attached screenshot called Short_answer_error.png. Please note that I have also set up the quiz module and short answer question type on the site http://dev.parenti.yale.edu/chaptire-1a, and I do not get an error message there (see attached screenshot called Correct_answer_user_answer.png). Finally, the former site also presents the user’s answer and the correct answer in a slightly different way than the latter, and it also includes a feedback textbox. Do you have any suggestions on how I could get the short answer question type on http://dev.variationsstylistiques.yale.edu/chapitre-1-quiz to function and display in the same was that it does on the site http://dev.parenti.yale.edu/chaptire-1a?

The client decided not to move forward with the short question type and the quiz module.
