Bassett award application link at ritm.yale.edu

Hello, 

I need to add the following code to the Bassett award application. Unfortunately, every time I add the code/link it just ends up showing on the page itself - instead of the actual application. I attempted to link a box in the application, but that also did not work b/c it is not an external URL or an internal path the code is from the Admissions office and this is a collaborative effort. I have a similar code for the nomination page and it works just fine. I can’t see what I am doing incorrectly.
