Adjust question format

Hello,

We recently requested two additional questions in our online parent sign up form. The added questions were “Race” and “Gender.” Could you please make the format for these questions match the format of all the other questions? E.g. it should not have the subquestion “Gender” and then within that question, a box also labeled “Gender.” If we could get rid of that extra box for Gender and Race without getting rid of the question comment that would be amazing! If the box needs to stay because the question is multiple choice, then simply getting rid of the extra “Gender” and “Race” label on the box would be great.
