Problem with CSV data download on site

Hi Yale sites helpers,

We have a form or worksheet, but the download CSV function for downloading the results is not working. It’s a button at the bottom of the page here: http://gsl.yale.edu/program-worksheets

When you click the CSV download button at the bottom, it whirs then comes up with this error message:

This page isn’t working

gsl.yale.edu is currently unable to handle this request.
