YSN Simulation Lab Room Request Form

Hi,

It seems that our faculty is having a problem submitting  room request forms for the Simulation Lab. Sometimes a request will go through fine but it seems when you request more than 1 room at a time it gives an error message.  Once you get the error message and try and go back in the rooms you requested everything is unchecked and it will not except if you pick one room or all the rooms. It just keeps coming back with the error code. Please advise on how to fix this.
