Assistance Needed ASAP

I need to make changes to our website that are very specific (too much for the message boards).  Can someone come here to help with this? It may also need to be done on your end. 

We need to insert a table at the bottom of the site, underneath Data Processing.  The size should be adjusted to match the general size/format of our webpage.

Then, underneath the table we need to insert a button titled “Download Data” that leads to this agreement form, along with the fillable boxes:

The “send mail” button should be changed to “Submit”.  If possible, we would like the form that people submit saved in a directory on the server.

Then the data would be available via link to download.  If possible, we would like the form and the data to be downloaded on our server, not on the World Bank’s.

I have the table and the data on my computer & can send it along to whomever can help with this.  Would like it completed by Wednesday the 23rd if possible.
