Having problem with webform creation tool

Hello Nancy, Josue and Marwa,

This is a relatively urgent request. When I go to create a webform, it gives me the last webform that Lalani Perry created, not the means to create it. Please see below. I’ve gone in to create the form several times and this is all I get. Please help. Thanks!
